Peruvian Maca: Benefits, Contraindications and How to Take It

For the Incas it was sacred food, reserved for warriors before battle and couples wishing to conceive. Today Peruvian maca is among the best-selling adaptogens in the world — but also one of the most misunderstood: some consider it a "natural viagra" (it isn't), others accuse it of raising testosterone (it doesn't), some fear thyroid contraindications (it depends).

What is Peruvian maca (Lepidium meyenii)
Peruvian maca, whose scientific name is Lepidium meyenii, is a plant of the Brassicaceae family — the same as broccoli, cabbage and radish. It's native to the central Andean plateau of Peru, where it has been cultivated for at least 2,000 years in the departments of Junín and Cerro de Pasco, at altitudes exceeding 4,000 metres. It's one of the few food plants able to survive the extreme temperature ranges, lack of oxygen and intense UV radiation of those altitudes.
The part used is the tuberous root, resembling a small radish. It's harvested after 7-9 months of growth, sun-dried for weeks and then reduced to powder, the traditional form of consumption. Maca has an earthy, slightly bitter and malted flavour — many add it to smoothies, porridge or coffee, but the form with documented clinical effects is the standardised extract in capsules.
Root chemistry: macamides, macaenes and glucosinolates
Maca contains many unique bioactive compounds. The most important are:

What is maca useful for: scientifically documented benefits
Contrary to exaggerated marketing, maca has concrete benefits supported by clinical trials — not all those you read, but those that exist are robust.
Libido and sexual function
This is the effect with the strongest evidence. The Shin 2010 systematic review (PMID 20691074), published in BMC Complementary and Alternative Medicine, analysed 4 RCTs concluding that maca has beneficial effects on sexual dysfunction in three populations: healthy men, men with mild erectile dysfunction and menopausal women with SSRI-induced low libido. The Gonzales 2002 study (PMID 12472620) on 57 healthy men showed that 1,500-3,000 mg/day for 12 weeks increases sexual desire without modifying testosterone, LH, FSH or prolactin. This is the crucial point: maca works on libido through a non-hormonal mechanism.
Energy and fatigue reduction
Maca is traditionally used as a tonic against physical and mental fatigue. Studies show improvements in perceived energy, reduction of chronic fatigue and better tolerance to exertion. The mechanism is probably adaptogenic: maca modulates cortisol response and supports the hypothalamic-adrenal axis.
Hormonal balance and menopause in women
Several RCTs (particularly Meissner 2005-2006 on gelatinised maca) have shown that maca reduces menopausal symptoms (hot flushes, insomnia, night sweating) and improves mood and libido in peri- and postmenopausal women. The peculiarity: maca is not a phytoestrogen, so it represents an option even for women who can't/won't take phytoestrogens (soy, red clover) — for example due to hormone-dependent breast cancer history. The mechanism acts on the pituitary axis, not on oestrogen receptors.
Memory and cognition
Studies in animal models and preliminary human studies suggest a neuroprotective effect of maca, with improvement in working memory and learning. The Gonzales 2012 review (PMID 21977053) summarises available studies. The mechanism probably involves acetylcholinesterase inhibition and antioxidant protection.
Male fertility
RCT studies have shown that maca increases ejaculate volume, sperm count and motility in healthy men. Effects are probably independent of testosterone, which as mentioned is not modified. Used as adjuvant in natural fertility support protocols.
How to take maca: dosages, times, formats
Effective dose according to clinical trials
The effective dose documented in clinical trials is 1,500-3,000 mg/day of root powder (equivalent to about 1.5-3 g). With concentrated extracts the amount is much lower: a 100:1 extract (where 1 g of extract corresponds to 100 g of root) allows doses of 100-300 mg/day with comparable effects. Studies with doses under 1 g/day of powder have unconvincing results.
Gelatinised maca, powder or 100:1 extract: what's the difference
Root powder: traditional format, but contains raw starch that some people digest poorly. Requires significant daily doses (2-3 g)
Gelatinised maca: powder passes through hot water at low pressure, eliminating starch. Improves digestibility and concentrates active ingredients. Many clinical trials use this form
100:1 extract: root is extracted with solvents (water or alcohol) to concentrate macamides 100 times. Allows very reduced doses (100-300 mg/day) with equivalent or superior effects to 2-3 g of powder
Recommended cycle
Maca works in the medium-long term. First effects (energy, mood) are noticed typically in 2-4 weeks; the effect on libido requires 8-12 weeks to manifest consistently. The standard protocol is 12 weeks of continuous intake, followed by 4 weeks off. There's no evidence of tolerance or habituation, but the cycle with break allows assessment of the real benefit.
Time of day: many studies use it in the morning or split morning/lunch to not interfere with sleep (some find it mildly energising). Empty or full stomach: both options are valid, depending on personal gastric tolerance.
Contraindications: who should NOT take maca
Pregnancy and breastfeeding
There are no safety studies in pregnancy and breastfeeding. As a precaution it is not recommended in these phases, as for most plant supplements.
Uncontrolled thyroid disorders
Maca contains glucosinolates (sulphurous compounds typical of Brassicaceae) which, at high doses and with iodine deficiency, can interfere with thyroid function. In practice the risk is very low at therapeutic doses (1.5-3 g/day) in people with adequate iodine intake, but those with Hashimoto's hypothyroidism or on levothyroxine should consult the doctor and evaluate possible TSH monitoring. Gelatinised maca has reduced glucosinolate content and lowers risk.
Drug interactions
Maca has no major known interactions, but caution with therapy involving:
Anticoagulants (warfarin, DOAC): INR monitoring
SSRI/SNRI: maca can improve sexual dysfunction from antidepressants but don't stop therapy
Hormonal drugs: contraceptives, tamoxifen, replacement therapy — talk to the doctor
Hormone-sensitive tumours
Although maca contains no phytoestrogens, in case of hormone-dependent neoplasms (breast, endometrium, prostate) consultation with the oncologist is appropriate before taking.
Black, yellow and red maca: which to choose
Maca comes in three chromatic varieties depending on root pigment:
Yellow maca: the most common (60-70% of production). General effects on energy, mood, libido of both sexes

FAQ — Peruvian maca
Does maca increase testosterone?
